Does my insurance cover services?
Each insurance plan varies. Each therapist’s profile lists the insurance they accept. If your insurance is accepted, you can call your insurance to check benefits/eligibility. Please do this prior to your initial appointment. We can provide super-bills as out-of-network providers for other insurance companies. If you have BCBS through AHCCCS, counseling services are not covered at FCC. Make sure to verify your benefits to avoid outstanding charges.
What payment types are accepted?
We accept cash, check, debit card, credit card, and HSA/FSA card. Please complete the payment authorization form to put a card on file with your individual therapist.
Can I get a statement to show my payments or balances?
Yes. You can ask your therapist directly for a print out or email version. You can also email Vertical Solutions at billing@flagcounseling.com.
What are your Private Pay rates?
Intake sessions are $200. This is the initial session including assessment and diagnosis.
50 Minute individual sessions are $150 each.
50 Minute family/couples sessions are $150 each.
* If you are uninsured and planning to self-pay or are insured and planning to not use your insurance toward the costs of therapy, you are entitled to a Good Faith Estimate.
How do I pay a current or past due balance?
You can email or call your therapist directly. If you have a card on file, it can be charged. If you want to pay directly, please arrange a time to meet with your provider. A check can be mailed to Flagstaff Counseling Center, 408 N Kendrick St. #4, Flagstaff AZ 86001.

What if my insurance does not cover services?
If your insurance is not accepted, you will be charged the private pay rates. Billing statement and super-bills can be printed for your records. You are financially responsible for any services rendered. Verify your insurance benefits before scheduling an appointment.
*Payment is due at time of service.
Appointments cancelled within 24 hours and no-shows have a $75 fee.*
